Lloyd Fisher, MD, FAAP W ith so many issues and problems going on in the world today, some- times we, as pediatricians, eel over- whelmed at the prospect o trying to make a dierence in the lives o our young patients and their amilies. I o- ten eel that I am really being asked or even expected to truly “fx” entire lives. Though my partners, ofce sta, and I do attempt to keep to the goals o being a medical home, we are oten unable to really mend these amilies’ complex and severely broken lives. Even sticking to the purely health-related issues (such as giving anticipatory guidance about saety issues, eating a healthy diet, and getting regular physical activity) can prove challenging depending on the amily with whom we are working. I recently had an experience in my ofce where I realized that, sometimes, even a small project can have a major positive outcome. I’ve become increas- ingly rustrated with an all too common situation in my ofce. I’ll be discussing with a parent and child the concerns I have about the child’s weight, poor eat- ing habits, and lack o physical activity. All the while, the child is sitting on the exam table, eating a bag o chips or some other very unhealthy snack. I not or the signifcant negative eects o such regular consumption, the irony o this is almost amusing. It is quite typi- cal or a parent to tell me that his or her child never eats “junk ood” while that very same child has residue rom the snack all over his or her ace. I was horrifed to discover that chil- dren were obtaining these snacks rom the vending machine every patient must walk by when going rom the registra- tion desk to the waiting room. What kind o message are we sending to these children and their caregivers? I dis- cussed this with my partners, who all agreed we should have the vending ma- chine removed rom the ofce or, at the very least, have the vending company alter the selection o snacks to include healthy choices. This was a seemingly simple solution to a problem that, un- ortunately, became ar more complex. It turns out that my practice signed an agreement with the vending compa- ny 20 years ago. The contract automati- cally renews every fve years unless we contact them within the two-month pe- riod prior to the renewal date. The con- tract specifcally states the location o the machine and that the company has the authority to determine what is in the machine. The administrator who initially signed this contract is long gone, and nobody around had any knowledge o such a contract until we contacted the company to remove the machine.
