T [ presidenT’smessAge ] he most signicant Patient Protection and Aordable Care Act (ACA) changes to the health care industry (including Health Care Exchanges) will be implemented this January. Signed into law by President Obama in March 2010, ACA enables open enrollment of beneciaries into the Exchanges beginning October 1. Health Care Exchanges are an online marketplace where patients can shop around for health care plans, just like auto insurance. All 50 states will have their own marketplaces, some of which will be run by the federal government and some of which will be run by individual states. Families and individuals who earn less than 138 percent of the federal poverty level will continue to have access to Medicaid. At best, Medicare coverage in states will be expanded under “Obamacare.” At worst, states will exercise their right to skip an expansion in favor of the status quo. So far, about 13 states have opted out of an expansion. So, yes, we anticipate you will face confusion and challenges posed by patients and “insurance plans.” And, yes, government actions may be even more complicated and frustrating than the implementation of Medicare Part D at the end of the discount-card phase. erefore, AAP is working proactively to create informational documents and reference libraries to help educate members about aspects of the law that will impact patient populations as well as daily pharmacy business practices. No doubt you will be stymied by many dierent ACA-related questions from patients. ings like what is the dierence between a silver, gold or platinum plan? At what point do I qualify for these government subsidies that I have heard about? When does the requirement for insurance issuers to stop excluding beneciaries with pre-existing conditions go into eect? What’s the best website for ACA information? We are planning a series of fax broadcast and electronically-available information sets for our members. Additionally, the Henry J. Kaiser Family Foundation has done a very thorough job of explaining multiple aspects of the law at www.k.org. And the federal government has established www.healthcare.gov; though at the time of this publication, much of the site’s information was somewhat promotional. ere’s no denying it. e pharmacy game has changed dramatically compared with “the good ole days.” Today’s rigorous playing eld is lled with increased complexities, lower margin percentages, and greater government regulations and compliance mandates (topics we address in this issue of VO1CE magazine). ese days, essentially all American lives are covered by third-party/government plans. And independent pharmacies are facing all of these game changers while attempting to skillfully meet an ever-increasing demand for more patient services. AAP is here as your Defensive Team.
