Our health services in North Tyneside and health and social care services in Northumberland look after the wellbeing of a population of around half a million people. Hospital services are provided from three modern general hospitals – Hexham, North Tyneside and Wansbeck –as well as community hospitals throughout Northumberland. We also care for people in their homes, or as close to their homes as possible, and provide services from facilities in local communities, such as health centres. Wherever possible we support communities to take positive steps to improve their health and prevent illness. We encourage individuals to stop smoking, increase physical activity,improve their diet and maintain positive mental health and offer services to support them to do this. We manage adult social care services on behalf of Northumberland County Council, helping to ensure people move between hospital, community health and social care services easily and with continuity of care. We also give people greater choice and control over their care to help them to live independently at home and to avoid hospital admission where appropriate.
